Toolkit and resources
This toolkit was designed in consultation with service users, their families and whanau, clinicians and managers. It has been in use by some district health boards in New Zealand since 2002. It is designed for use by primary and secondary care providers in New Zealand.
The toolkit comprises a Microsoft Excel spreadsheet. Inside it you will find:
- instructions for using the toolkit, including the getting started guide
- information on the 10 key features of good service provision
- the service benefits checklist, which gives a baseline impression of how well your service or team is currently providing the 10 key features
- manual data collection form
- data entry page
- summary report - showing the results for your service or team
- service user's report (you can print this page to provide to service users with their KPP information).
