Knowing the People Planning toolkit

Knowing the People Planning is a tool designed to help service users, family, whā nau and services identify the things that will help someone address their recovery in the best way possible. This toolkit was designed in consultation with service users, their families and whanau, clinicians and managers. It has been in use by some district health boards in New Zealand since 2002.
In 2010 the toolkit was redeveloped again based on feedback from the sector and trialled for six months before release in June 2011. The improved toolkit offers real time access to KPP information that can be used at clinician, team and service levels. To enhance accessibility it has been available for both Excel 2003 and 2010 and is free to download.
