Suggesting a story of change

The online expression of interest process consists of four steps where we collect information. The final step allows you to review the details you have entered before making your submission.

It is helpful to prepare information before you begin your submission - check out the step by step guide below to see what questions are asked and what you will need to know.

Step one - administrative details

This collects administrative details such as the name of your initiative and contact details for the key contact and organisation.

In this step we ask you what region of New Zealand the initiative is located in, as follows.

  • Northern (Northland, Waitemata, Auckland and Counties Manukau DHB regions).
  • Central (Whanganui, MidCentral, Hawke's Bay, Capital and Coast, Hutt Valley and Wairarapa DHB regions).
  • Midland (Taranaki, Waikato, Lakes, Bay of Plenty and Tairawhiti DHB regions).
  • Southern (South Island DHB regions).
  • If your service or initiative crosses more than one region please select ‘national'.

Step two - initiative or service details

Here we ask the following questions to learn more about your initiative. Questions have either yes or no answers or allow you to type your answer into a free text box.

  • The year the initiative started.
  • Status - development/trial or implemented.
  • Describe your service.
  • Why was this initiative (service, project) necessary?
  • Describe the background, any research or evidence that supported the development of this initiative.
  • How are service users involved in this initiative/service?
  • Has the initiative been evaluated? (If yes, we ask for the date of the last evaluation.)
  • What are the key outcomes of the initiative?
  • What are the three key learnings to share with others hoping to provide something similar?
  • Funding source(s).

Step three - key descriptors

Here we collect information that helps classify your story for search purposes. There are drop down boxes where you can select key words/descriptors that match your initiative most closely and add them to your submission. Please select all of the descriptors that apply to your initiative - this will ensure it is readily found when people search the stories of change.

If you can think of other key words/descriptors that describe your initiative you can type these into the ‘other keywords' free text box at the bottom of the page.

The categories we have are as follows:

  • key descriptors (general description)
  • disciplines (what types of roles are involved in the initiative)
  • settings
  • diagnostic categories
  • gender specific
  • ethnic or migrant or refugee specific
  • other.

Step four - supporting information

Here you can upload files to support your story suggestion, for example publications, flyers or evaluation reports. Click ‘browse' to locate the file on your computer and then type a title for the file into the box below. The file will be uploaded when you click ‘next'. If you would like to upload more than one file please click 'upload another file'.

If there is information available about your initiative on another website (such as your own organisation's website) you can add a link here, along with a name or brief description of what can be found on this web page. If you would like to add more than one link click 'add another link'.

Step five - review your submission

On this screen you will be able to see everything you have entered so far. If you would like to amend your submission you can go back and make changes.

You can print the review page for your records if you wish. You will also receive a copy of this information to the email address you entered in step one as confirmation of your submission.

The submission of expression of interest does not guarantee that your story will be published. For the purposes of publishing on our website, Te Pou reserves the right to edit this copy. If we develop your story we will ask you to verify the story for accuracy before it is published on the web.
