About the team

Based at Wakari Hospital the South Community Mental Health Team provide care, support and treatment for people experiencing moderate to severe mental illness.

About the role / About you

We are looking for a flexible and motivated team member to work within the Clinical Needs Assessment (CNA) team.

Your role will be to assess clients in a variety of health settings and create a plan of care that is client centred and goal focused including developing support packages in a holistic and culturally appropriate manner for adults who experience mental health issues and require support in the community.

You'll be working with people and their family/whanau to achieve the best outcomes for them. You will also need a high level of verbal and written communication skills and experience working with electronic systems and be confident using a computer.

You will need to be a registered health professional under the Health Practitioners Assurance Act or Social Work Registration Act and hold a current practising certificate. You need to have excellent interviewing, assessment, problem solving and networking skills.

Ideally you will have experience working in a mental health setting with people in either a hospital or community. You will need an excellent knowledge of community resources and services that are available to the person.
