The details: Permanent – 55 hours per fortnight
We have an exciting opportunity for an experienced Community mental health nurse, Occupational Therapist or Social Worker to be a part of service development and strengthening the way we support our community.

About the role:
The ICCT team works alongside the Adult Mental Health Triage Team. ICCT supports the transition of Tangata Whaiora from mental health services back to primary/ G.P care and provides brief interventions to Tangata Whaiora referred into service via Triage, whose needs are best met on a short term pathway. The role has a strong emphasis on consult liaison, relationship development, brief interventions and wrap around support.
Required skills & experience:
You will be an experienced and dynamic mental health professional that is can provide therapeutic intervention, brief intervention and coordinate care.
You will provide specialist advice to G.P’s and practice nurses around the mental and physical health needs of Tangata Whaiora.
You will be confident providing education and liaising with a wide range of people and services and negotiating shared care arrangements.
You will have a strong commitment to promoting positive relationships, quality improvement and collaboration between all key stakeholders.
