About the role

Our newly created High Impact Support team will work within a multidisciplinary framework to facilitate better access for telehealth service users with complex and/or enduring physical or mental health needs to clinical and/or self-management support. These service users will be identified by their pattern of service utilisation and/or internal staff referral.

About you:

  • Current registration with an appropriate professional body within Healthcare
  • Current Practising Certificate
  • Post graduate education in Health
  • Post-grad experience working within a multidisciplinary environment
  • Knowledge and skill establishing therapeutic relationships in a virtual environment
  • Deep understanding and knowledge of community needs, resources and culture, both regionally and nationally
  • Ability to liaise, coordinate and maintain positive relationships across internal and external stakeholders including multiple providers
