This includes completing client assessments, creating and reviewing goal-based service plans, and providing staff training and clinical oversight where required.
You will have experience working within a goal-based model in the community sector and will be driven by a passion to deliver exceptional restorative services to people with health and disability related needs in their own homes, with a focus on those over 65 years
What you will bring
The successful candidate must hold a current annual practicing certificate (APC) with a relevant regulatory body and a clean full NZ driver's licence.
