Registered Social Worker - MHSOP

We have an exciting opportunity for a Registered Social Worker with Mental Health experience to join our Mental Health Services for Older People (MHSOP) Community Team. This wonderful service provides specialist diagnosis assessment and treatment, specialised dementia support for behavioural and psychological symptoms of dementia and complex dementia and tailored plans to keep clients safe and well in their homes.

We also have a memory clinic for individuals who require diagnostic clarity around complex early memory problems.

The focus is to deliver high quality care to the growing population in the Bay of Plenty.

You will be working with a multi-disciplinary team consisting of other Social Workers, Psychiatrists, Psychologists, Occupational Therapists, and Nurses. You will work with referrers, health partners, referred clients, their families/whànau and the community to provide an effective triage and assessment, to facilitate and implement planned treatment to maximise health and independence in the community:

  • Provision of advice and assistance
  • Resolution of the mental health crisis
  • Facilitating transition along the care pathway for service-users and their families
  • Linking consumers to primary care providers and appropriate community agencies

The successful applicant will be required to have an excellent understanding of mental health conditions, a good understanding of dementia, the Mental Health Act, PPPR Act, understanding of capacity assessment work up, and a range of therapeutic modalities. You will be professional, empathic, a good team player and flexible.

We are looking for someone will a minimum of three years post graduate Mental Health experience, and preferably experience in older person’s mental health. This position is based in Tauranga where you will be working in both inpatient and community settings.
