HoNOS outcome graph builder tools for DHB PRIMHD outcome reports - team

These tools are designed for DHB site coordinators and DHB outcome champions.
Te Pou provides each DHB with PRIMHD outcomes summary reports every three months.
- this information can used to build a graph with the use of these tools
- the graphs visually display up to four individual team’s information at a time
- the graphs could be used in team or management meetings to review and discuss information and progress about outcomes reporting.
