The support worker’s manager or lead completes the registration form below. On receipt of the registration Te Pou will email the health coach/support worker requesting they complete and submit a form to complete the application process.

Te Pou will confirm allocation of a training space to the support worker’s manager or lead.

Please tick the box below to confirm that the person you are nominating is aware of their nomination.

Please tick to confirm that as the manager, the nominated support worker will be able to provide supervision to others as part of their current role.
