Equally Well Organisation and service self-assessment tool

The aim of this self-assessment tool is to assist providers to identify how they are currently supporting the physical health needs of people accessing their organisation and services.

The use of the tool will identify potential areas for development to support equitable physical health outcomes for people with mental health and addiction issues. Where gaps in support are identified, organisations and services are encouraged to use local networks, eg partnering with DHB providers to carry out blood tests, to address these needs.
